Initial Comments:
An off-site follow-up survey conducted on July 11, 2022, found that Extracare Homecare Services, Inc., had corrected the following deficiencies cited under 28 Pa. Code, Health Facilities, Part IV, Chapter 611, Subpart H. Home Care Agencies and Home Care Registries. The deficiencies were cited as a result of an onsite state re-licensure survey conducted on May 10, 2022.
